When you are ready with the names and phone numbers of the people who agree to help you earn your gift, you can participate in one of two ways:
- Fill out this form, and give it to the representative who provided you with the form, or fax the form to 905.629.4638
- or
- Call us at 1-888-712-4400 and ask for Magda in the Referrals Department.
In order to qualify, your referrals will need to live in a house and need to be the homeowners, and the homeowners will have to either have a water test, or both a water test and installation through Lifetime Water Systems Inc. Names will need to be provided to Lifetime Water Systems within 14 days from the date you received this form. Providing the Terms and Conditions (see below) are met, the rewards will be awarded as follows:
- $200 Grocery Coupon* - for each five (5) completed water tests, or for the first installation (whichever comes first)
- $250 Grocery Coupon* - for the second installation
- $300 Grocery Coupon* - for each additional installation
